Q1) In your prof. experience, describe some common medical diagnoses that require 504 Plans. What are some helpful mods/accom?

A: Most recently: dyslexia. Lots of multi-sensory accom, use of iPad, testing accom

A1: fine motor issues - use of computer/ extra time for writing/consult w/ OT.
A1: ADHD - extra time or time constraints depending on Ss need, chunk and check assignments, motor breaks
Dyslexia is tied in w/poor handwriting so we use these mods often too, Speech to text is effective also

I like preferential seating also. Limits distractions
yes! I would say most of mine are related to ADHD and Anxiety too.
Yes... where the student is seated usually is addressed in our plans too.
One problem I have with preferential seating..what constitutes preferential? Teachers are always moving
A1: many times seated next to peer role model finds its way onto plans for Ss w/ ADHD.
I had one for a student with Tourette's Syndrome.
Wherever the S can most meet w/success despite the ADHD. For some this may mean moving seats often

A2: Each school has committee, District has supervisor, I only coordinate/help formulate 504 Plans for dyslexia cases

Has to be the S. Their needs are at the forefront.
I think it depends on the S. Some may be more successful in back of room, cuts out the constant turning around
think it depends on the S some need it near instruction to focus. some need to be seated away from other S to focus.
A2) Our nurse is responsible for coordinating w/ parents, teachers, counselors, etc. I am responsible for monitoring.
A2: our student support team is our 504 team. includes Ts, assistant Principal, counselor, parent, some times county nurse
A2: currently counselor takes notes and types up paperwork. soon our student support team coordinator will be responsible for this

A3: Communicate, revisit, reevaluate
. on an as-needed basis or at designated intervals?
Depends on case how often, but quick check-ins can't hurt
A3: understanding of the issue specific to that S, communication re: effectiveness of plans in regards to Ss success

I agree. Being informed on the issue is key. Helps Ts understand the purpose of the 504
