Earlier today: me = my nurse documented an iodine allergy, what does that mean? pt = everytime I even smell the iodine in a crawfish or shrimp I start swelling and can’t breath. #notanallergytoiodine#asktherightquestions#ACCFIT
Actually the bigger problem is medical education. In reality, a resident could never stand up to an attending - they'd be immediately disciplined, put on probation, and soon fired. Change in healthcare needs to start with change in #meded
The Idaho College of Osteopathic Medicine just announced that it has accepted its first set of med students for its inaugural class. Of the 18 accepted future doctors, 17 are from Idaho. The school intends to accept 150 first-year students total. https://t.co/7ZaU8nwmkf#MedEd
Absolutely. It seems that when schools do shorten/become more efficient, step 1 scores aren’t worse...but many barriers exist to shortening “pre-clerkship” curriculum. #MedEd
T1 #meded This is a challenge we're facing now. The resistance is being driven by pre-clinical faculty feeling that they can't possibly prepare #medstudents for #USMLE unless they're in class for 40 hours a week!
Stick to cutting ribbons please, not boys. Every nontherapeutic child circumcision is a gross violation of all 4 cardinal principles of medical ethics, and there are thousands of them in US every day. #tweetiatricians#tweetiatrician#i2@AAPNews#AAP18#meded#bioethics
Also financial implications (especially for small departments), change fatigue, “overlap year” that can over-saturate clinical preceptor capacity - to name some others. #meded
Topic 2: Now that we've talked about the benefits of shorter #meded, what happens if a #medstudent takes longer to complete school? Unintended consequences?
T2 #meded I'm curious to see what people think of this. Particularly if we are talking about #CBME. Does longer imply lack of ability? Will that hurt #medstudents in the Match?
So for #CBME to really work it’s going to have to go along with #USMLE reform as well. Until all are competent/not competent (ie pass fail) then those who are delayed will be harmed. #meded
Topic 3: What other unintended issues might arise for flexible paths for completing medical school? How might this impact residency programs? @acgme#meded
T3 #meded I think this is where the greatest challenge arises. Can residency programs accept #medstudents who complete in November of their final year? Won't budget constraints create a problem for #CBME to be effectively implemented? @acgme
Anyone interested in #meded, #QualityAssurance or #patientsafety needs to read this account. What an incredible system failure resulting in a young death.
Residents/trainees: it’s OK to ask for help, & speak up when you’re overwhelmed.
Having read this account, I would be lying if I said that I could have performed any better than Dr Bawa-Gaba. This could have been any of us. This is a travesty. A trainee is taking the blame for a system failing, and we wonder why aren’t filling posts? https://t.co/dJMYAPYC1j
T3- if residents start early, they likely will end early? What if there aren’t enough students ready to match/start? Service gaps? Can say it’s not right, but teaching hospitals depend on residents for patient care. #MedEd
T3 #meded That is reality. So a perplexing situation arises in that flexible completion times may not be as easily accomplished as we think.....particularly if it is not possible to have flexible entry into postgraduate training.

T3 #meded So do you think we should go back to more of an open market system for the Match? It would almost seem like you'd have to if every #medstudent was graduating at varying times
All of us in psychiatry know that PTSD makes forming healthy relationships difficulty. Many healthcare professionals “eat their young” driving #empathy right out of #practice. #MedEd#traumainformed

T3 I'm not sure how bad it would be. Might make the process more manageable and cost effective for #medstudents to not have to go through that process. Could curtail getting into programs perceived to be more competitive.
#meded agreed. bottom students don't match to top programs & vice versa; it's the point of the match. big challenge would be interviews, travel etc; maybe internet round one, in person round two?
